The RPM Program involves using technology to monitor your health data remotely. This includes collecting and analyzing vital signs, activity levels, and other health metrics using devices provided or approved by your healthcare provider. The goal is to detect changes in your condition early and provide timely interventions.

Yes, you can opt out of the CCM or RPM programs at any time. Simply notify your healthcare provider in writing of your decision to opt out, and we will terminate your participation in the Program. Your health data collection will cease immediately upon opt-out.

Opting out of the CCM or RPM programs will not affect your access to standard medical care from your healthcare provider. You will continue to receive routine care and services as usual.
