Chronic Care Management and Remote Patient Monitoring Program

Welcome to the WellnessWits Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) Program (the "Program"). These Terms of Use (the "Agreement") govern your use of the Program provided by WellnessWits ("we," "us," or "our"). By accessing or using the Program, you agree to be bound by this Agreement. If you do not agree to these terms, do not use the Program.

Definitions 

  • Chronic Care Management (CCM): CCM refers to the coordinated care and management of chronic conditions for patients, involving regular check-ins, care coordination, medication management, and personalized health plans. The goal is to improve overall health outcomes and quality of life for patients with chronic diseases. 

  • Remote Patient Monitoring (RPM): RPM involves the use of technology to monitor patients' health data remotely. This includes the collection and analysis of vital signs, activity levels, and other relevant health metrics using devices provided or approved by WellnessWits. RPM aims to detect changes in a patient's condition early and provide timely interventions.

Program Description 

  • Chronic Care Management (CCM): As part of CCM, WellnessWits provides personalized care plans, regular check-ins with healthcare providers, medication management, and coordination with other healthcare services. The CCM program is designed to help manage your chronic conditions effectively and improve your health outcomes. 

  • Remote Patient Monitoring (RPM): The RPM program includes the use of devices to monitor your health data remotely. This data is transmitted to WellnessWits healthcare providers for continuous monitoring. RPM aims to provide timely insights into your health and enable proactive management of your conditions. ​​

Eligibility

Patients must have a qualifying chronic condition and receive a prescription for CCM-RPM services from their primary physician or a qualified healthcare professional.​​ Patients will be supplied with remote monitoring devices capable of daily recording and programmed alert transmissions.

  • Devices must be used for at least 16 days within a 30-day period.

  • Patients should have the physical and cognitive ability to use the RPM devices and technology effectively. Support from a caregiver may be required if the patient needs assistance.

  • Patients must not be enrolled in another RPM program that provides similar services. 

  • Patients must provide informed consent for the collection, transmission, and use of their health data as part of the RPM program. 

  • Patients must have one or more chronic conditions that require regular monitoring.

 

Patient Responsibilities

  1. Device Usage:

    • Use the provided remote monitoring devices as instructed for at least 16 days within each 30-day period.

    • Ensure daily recording and transmission of vital sign data.

  2. Communication:

    • Report any issues with the device or data transmission to the healthcare provider immediately.

    • Attend any scheduled follow-up appointments or virtual check-ins as part of the RPM program.

  3. Adherence:

    • Follow the care plan and usage guidelines provided by the healthcare provider to ensure compliance with the CCM-RPM program requirements.​​​

  

Program Benefits  

  

CCM-RPM services allow your healthcare provider to monitor your health on an ongoing basis in between visits. This may help your healthcare provider identify issues that need to be addressed sooner than they would without RPM and allows you to communicate your information to your provider without having to travel to your provider’s office. RPM relies on devices and a reliable Cellular and Bluetooth connection to transmit data, and deficiencies in connection may result in missed readings or failure to transmit information. 

  

By providing your consent, you acknowledge the following: 

  

  • Your physician or other healthcare provider has explained to you what RPM means, the type of health data that will be collected, and how it will be used in your care; 

  • You are aware that your health data will be collected and digitally transmitted from an RPM technology to your healthcare provider in a safe and secure manner to maintain the confidentiality of your healthcare information; 

  • You will not transmit or allow to be transmitted the health data of any individual other than your own; 

  • You will not intentionally tamper with any RPM device used in connection with your RPM services; 

  • Your physician or healthcare provider is not responsible for inaccuracies in the health data transmitted; 

  • You consent to the use of RPM services as part of your care and treatment; 

  • You have the right to withdraw this consent at any time; 

  • You are responsible for all applicable copay and deductible amounts (including, if you are a Medicare beneficiary, the 20% copay for Part B services); and 

  • RPM services are NOT emergency services and your data WILL NOT BE MONITORED 24/7. If you think you are experiencing a medical emergency, CALL 911 IMMEDIATELY. 

  • WellnessWits is in no way acting as a medical provider. The ultimate responsibility for diagnosing and treating you rests with your healthcare provider. 

  • You agree to hold all members of WellnessWits harmless from and against any damages, liabilities, penalties, losses, costs, and expenses relating to any claim or action relating to WellnessWits’s services or devices. 

  

By providing your verbal consent to a physician or physician’s staff member, you acknowledge that you have read/verbally received and understand all of the above and that you consent to receive RPM services from your healthcare provider. You also understand that a physician or physician’s staff member will provide a digital attestation of your consent for record keeping.
